Make an Appointment
When to Call
Clients can call at 8:30 am Monday – Friday to schedule their same day appointment.
If you need a refill, pregnancy test or are concerned about exposure to a sexually transmitted
disease or infection, you can come in anytime between 8:30-5:30 as a walk-in. Please remember
that if you are a ‘walk-in’ your wait time can vary.
We are closed all major holidays.
What Should I Bring?
All clients are asked to bring a photo identification (drivers license, student identification)
AND their birth certificate, passport or Certificate of Naturalization.
Because Family Planning offers several programs to help with the cost of healthcare, you are also
asked to bring your current pay stubs to help us complete a financial assessment.
- Registration Forms
You will need to complete this form, please complete and submit it online or print it out and
bring it with you.
- Consent Forms
You will be asked to sign these forms, please take some time to review them before your appointment.
- Birth Control Consents
Depending on the method of birth control you are interested in, you will be asked to sign one of the
following forms. Please take some time to review the Birth Control Consent that you feel is appropriate
for you. If you have any questions about the consents, we can answer them for you when you come in.
